This site does not allow you to enroll online. To enroll, look for your Benefits Enrollment Form and 2010 Benefits Enrollment Guide that have been mailed to your home address. Benefit choices you make now become effective Jan. 1 and remain in effect for all of 2010.

Don't forget to return your completed and signed benefits enrollment form to your local HR department by Nov. 6.

All full-time U.S. employees of Enterprise Holdings who will be eligible for company benefits effective Jan. 1, 2010, will receive a Benefits Enrollment Form and Benefits Enrollment Guide at their home address in mid-October. To enroll, you must complete, sign and return your Benefits Enrollment Form to your local HR department by Nov. 6.

How to enroll

1. Read the Benefits Enrollment Guide
Take some time to review the benefits information on this site and carefully read the Benefits Enrollment Guide that was mailed to your home along with your Benefits Enrollment Form. You will find descriptions of your benefit choices, plan changes and premium rates for 2010.

2. Complete Your Benefits Enrollment Form
Your Benefits Enrollment Form is generated just for you. It shows your name and address, the benefit plans you are currently enrolled in, other options that are available to you and your dependents and the cost of each coverage option. When you feel comfortable with your benefit decisions for 2010, simply check the corresponding boxes on your Benefits Enrollment Form. It is important that you review your personal information and make any changes or corrections, supply all required information for any dependent you wish to cover under your benefits, including their Social Security number, and be sure that you have listed beneficiary designations for your life insurance benefit. Remember, if you want to participate in a Flexible Spending Account (FSA) for dependent care and/or health care, you must enroll each year and elect your annual election amounts. Prior elections will not carry over to the next plan year.

3. Return Your Benefits Enrollment Form
Submit your completed, signed Benefits Enrollment Form to your local HR department during the Oct. 19 - Nov. 6 Open Enrollment period. Your local HR department will enter your selections into the system, generate a Confirmation of Benefits Elections statement and mail it to your home address. Upon receipt, verify your benefit choices and notify your local HR department of any discrepancies by the date indicated on your statement. Benefits elections become effective on Jan. 1, 2010. Payroll deductions for benefit premiums begin in January 2010.

ID Cards
You will need to carry separate ID cards for Medical, Prescription Drug, Dental and Vision benefits. If you are enrolling in health care benefits for the first time, you will receive your medical, prescription drug, dental, and vision ID cards in the mail from each vendor in early January. If you are currently enrolled in health care benefits and will continue coverage in 2010, continue to use your current ID cards from UnitedHealthcare, Delta Dental of Missouri, and EyeMed. You will receive new prescription drug ID cards in the mail from Express Scripts in early January.

Tobacco Rates
There are two different premium rates for the medical plan. During Open Enrollment, elect the tobacco rate if you or any family members you plan to cover under the medical plan use any tobacco products. If you and your family members do not use tobacco products, elect the discounted non-tobacco rate.

To help tobacco users qualify for the discounted non-tobacco rate, we are pleased to offer CIGNA Quit Today, a tobacco cessation program through CIGNA Behavioral Health, available at no cost to employees and eligible dependents 18 years of age or older. Tobacco users enrolled and participating in this program by Nov. 6, 2009 will qualify for the lower premium rates effective Jan. 1, 2010. Adjustments will be made once we receive the participation report from CIGNA. Participation in the CIGNA Quit Today program will be monitored throughout the year. Those failing to actively participate in the program will be transferred to the tobacco rate midyear.
