- Choose health care coverage from the options listed on your Benefits Enrollment Form
- Options may include: PPO300, OOA300, PPO500, OOA500 or EPO (not all plans are available in all areas)
- Enroll eligible dependent(s) in your same plan choice
- Select Primary Care Physician (PCP) (for each EPO participant)
- WAIVE health care coverage (include source of other)
- Enroll for vision benefits
