Benefit What you need to do

Health Care Insurance


  • Choose health care coverage from the options listed on your Benefits Enrollment Form
  • Options may include: PPO300, OOA300, PPO500, OOA500 or EPO (not all plans are available in all areas)
  • Enroll eligible dependent(s) in your same plan choice
  • Select Primary Care Physician (PCP) (for each EPO participant)
  • WAIVE health care coverage (include source of other)
  • Enroll for vision benefits
  Note: The UnitedHealthcare, Delta Dental and Express Scripts benefits described on this site DO NOT APPLY to Hawaii employees. Go here for Hawaii Health and Wellness.

  • Health Care Spending Account
    ADP Benefit Services

  • Dependent Care Spending Account
    ADP Benefit Services

  • Enroll for Flexible Spending Accounts.
  • Select annual contribution amount for health care spending account (up to $3,000/year for 2006).
  • Select annual contribution amount for dependent care spending account (up to $5,000/year for 2006).
  • WAIVE participation in Flexible Spending Accounts

Setting aside pretax dollars can reduce your taxes and increase your spendable income.

  • Sign up for employee Optional Life Insurance.
  • WAIVE employee Optional Life Insurance.
  • Enroll your spouse or child(ren) for Dependent Life Insurance.
  • WAIVE spouse or child(ren) Dependent Life Insurance.
