Life-Changing Events
During your employment at Enterprise, you may experience life-changing events that allow you to change your benefits elections. Examples of life-changing events include getting married, having a child or getting divorced. Relocating to another part of the country or a transfer to another branch location may require you to select a new health care provider. Switching from part-time to full-time employment status will open eligibility for our comprehensive benefits package and require first-time enrollment. When these life-changing events occur, you will have an opportunity to make certain adjustments to your benefits within a specified period of time.

Enrollment Process
- You are required to contact your local HR department in writing by e-mail, letter, or fax within 31 days of the life-changing event to complete the notification process. Once your local HR department has documented your notification, a Benefits Enrollment Form will be sent to your home address.
- Complete the new Benefits Enrollment Form that is sent to your home address, sign and return it to your local HR department by the due date printed on the form.
- Submit proper documentation to support the life-changing event with your Benefits Enrollment Form. Keep a copy of all forms and documentation for your records.
When will these coverage changes go into effect?
Provided you contact your local HR department in writing by e-mail, letter, or fax within 31 days of the life-changing event and submit a completed Benefits Enrollment Form and supporting documentation within the time allowed, eligibility for coverage will be as follows:
To Add Coverage
For some life-changing events to add coverage, benefits become effective on the first day of the next full benefit coverage period after you contact your local HR department and complete the notification process. Such events include:
- Marriage
- Divorce, legal separation, or annulment
- Spouse's or dependent's employer's plan decreases coverage
- Open Enrollment under a spouse's or dependent's employer's plan
Changes to premium deductions become effective with the first full paycheck after the coverage effective date.
For other life-changing events to add coverage, benefit changes become effective on the date of the event. Such events include:
- Birth, adoption, or placement for adoption
- Loss of employment of a spouse or dependent
- Change in employment of a spouse or dependent
- Qualified medical child support order
- Spouse or dependent moves into coverage area
Changes to premium deductions become effective with the first full paycheck following the date of the event. You are responsible for paying premiums retroactive to the date of the event.

To Drop Coverage
For some life-changing events to drop coverage, benefits will end on the last day of the benefit coverage period in which you contact your local HR department and complete the notification process. Such events include:
- Marriage
- Legal separation
- Spouse's or dependent's employer's plan increases coverage
- Employee or dependents become eligible for Medicare
- Open Enrollment under a spouse's or dependent's employer's plan
Changes to premium deductions become effective with the first full paycheck after the coverage effective date. The last premium is payroll deducted from the paycheck in which you contact your local HR department and complete the notification process.
For other life-changing events to drop coverage, benefits will end the day after the event to ensure all medical claims for the deceased individual are eligible for coverage. Such events include:
- Death of a spouse
- Death of a dependent
For other life-changing events to drop coverage, benefit changes are effective on the date of the event. Such events include:
- Divorce or annulment
- Loss of employment of a spouse or dependent
- Change in employment of a spouse or dependent
- Qualified medical child support order
- Dependent loses coverage due to marriage
- Spouse or dependent moves out of coverage area
If you neglect to contact your local HR department to complete the notification process and submit a completed Benefits Enrollment Form and supporting documentation within the time allowed, you will have to wait until the next annual Open Enrollment to make changes to your benefits and coverage changes will not go into effect until January 1st of the following year.
Tax Exemptions
Following a life-changing event you may also want to change your tax exemption status.
Beneficiary Changes
Following a life-changing event you may want to change your beneficiaries. Use this form to update your life insurance beneficiaries. You must designate the same beneficiary for Life Insurance (Basic), AD&D and Optional Life Insurance.
To update beneficiaries for your Retirement Savings Plan, visit the online beneficiaries service available through Fidelity NetBenefits® at www.401k.com. You may also request a beneficiary designation form by calling Fidelity at 1-800-835-5095.

Same-Sex Domestic Partner Benefits
You may enroll your eligible same-sex domestic partner and his or her dependent children in the benefit plans when you are first benefits-eligible as a new hire, when your employment status changes from part-time to full-time or during the annual open enrollment period. There are also certain life-changing events that may qualify for a midyear change in coverage.
To start the enrollment process, you will need to complete the Same-Sex Domestic Partner Affidavit and, in some cases, the Qualified Tax Dependent Affidavit. Then contact your Human Resources department or the Corporate Employee Benefits department for additional enrollment instructions.
To terminate the Same-Sex Domestic Partner Affidavit due to a life-changing event and drop coverage for a same-sex domestic partner and his or her dependents, you will need to complete and return the Statement of Termination of Same-Sex Domestic Partnership.
