Address Change or Relocation
Change of address due to a job transfer or the purchase of a new home may require you to make changes to your benefits. It is important that you make the necessary adjustments so you and your family members can continue to receive uninterrupted medical care once you are settled in your new location.
Simple Change of Address
Notify your local HR department with your address change immediately.
Relocation
Moving to a new area or out of town due to a job transfer or promotion may create the need to update benefit elections.
Medical Coverage

Medical plan options are determined by your home zip code. If a move to a new address results in a change to medical plan options, the Corporate Employee Benefits department will mail a Benefits Enrollment Form to your home address within 1 to 2 weeks of the address change. You must complete and return the form to your local HR department by the due date printed on the form.
If your medical plan options change and you do not return your Benefits Enrollment Form:
- Your coverage will default to your current plan at the same level of coverage (i.e., employee only, family, etc.) if it is still available; OR
- Your coverage will default to the PPO600 or OOA600 (out-of-area) plan at the same coverage level if your current medical plan is no longer available
If your medical plan options do not change as a result of your new address, you will not receive a Benefits Enrollment Form at your home address
and no changes to your benefits will be allowed.
Depending on your change of address the following may occur:
- Your new zip code may offer a medical plan option that was not previously available to you.
- Your new zip code may limit your medical plan options.
- If your change of address keeps you within the boundaries of the medical plan you are currently participating in, you must continue to participate in that plan. You will not receive a Benefits Enrollment Form and no changes to your benefits will be allowed. A change of address does not necessarily qualify as a life-changing event. Finding A New Doctor
Here are two ways to help you make a smooth transition to new health care providers and ensure uninterrupted medical care for you (and your dependents)
if you relocate.
- Link to www.myuhc.com to conduct a search for a provider in your new area.
- Call UnitedHealthcare at 800-520-0746 and ask a customer service representative for assistance in locating a new provider.
Dental Coverage
Relocating does not affect your dental coverage, so a new dental ID card is not necessary. However, you may need to find a participating dentist that is more conveniently located to your new address.
Prescription Drug Coverage
Relocating does not affect your prescription drug coverage, so a new ID card is not necessary. However, you may need to find a participating pharmacy that is more conveniently located to your new address.
Vision Coverage
Relocating does not affect your vision coverage, so a new ID card is not necessary. However, you may need to find a participating vision care provider that is more conveniently located to your new address.
Dependent Care Spending Account (DCSA)
If relocation results in a change in the cost or need for childcare, you may need to adjust your DCSA elections. Qualifying changes to your DCSA
must be made by following the enrollment process.
