Where do I find EyeMed Vision Care claims forms for out-of-network claims?
EyeMed claims forms are available at 800-841-1220 or www.eyemedvisioncare.com and only necessary when visiting an out-of-network provider. If you are serviced by an in-network EyeMed Vision Care provider, simply pay your copay amounts.
I will be waiving medical, prescription drug and dental coverage for this year. Can I still enroll in the vision plan with EyeMed Vision Care?
Yes! You can enroll for EyeMed Vision Care even if you do not elect the packaged medical, prescription drug and dental coverage.
I have employee only coverage for medical, prescription drug and dental for this year. Can I enroll me and my spouse for the vision plan with EyeMed Vision Care?
Yes! You can enroll any of your dependents for EyeMed Vision Care even though they are not enrolled in the packaged medical, prescription drug and dental coverage. However, you as the employee also have to be enrolled in the EyeMed Vision Care, too.
How do I get a new ID card?
If you should lose your EyeMed ID card, you can call EyeMed's customer service at 800-841-1220 to request a new ID card. You should receive a new ID card in 7 – 10 business days.
Can I purchase both contacts and glasses in the same year?
An employee would actually be eligible to receive both the contact lens allowance and the frame allowance in the same benefit period. These two benefits operate independently of each other. If a member would choose to get a frame, lenses and contact lenses in the same benefit period, the best way for the member to get the maximum out of the benefit would be to use the frame allowance and the contact lens allowance. The member can then use the secondary purchase plan to purchase eyeglass lenses for the frames.
In this case, the employee would have a $120 allowance for the frames and pay 80% of any remaining balance over the $120. The employee would also use the $115 contact lens allowance and depending on the type of contact purchased (conventional or disposable), may have an additional 15% off the retail price if the purchase was conventional contacts. The employee would then use the secondary purchase plan for scheduled discounted rates on the eyeglass lenses. |
