How do I register on the myuhc.com web site?
You can access the UnitedHealthcare web site by visiting www.myuhc.com. If you are not yet enrolled in a UnitedHealthcare plan, use ERAC for both the user name and password. If you are already enrolled in a UnitedHealthcare plan, follow the prompts under "Register Now" and use the member information as it appears on your ID card to register on www.myuhc.com. To reach UnitedHealthcare customer service, call (800) 520-0746.
I have health care coverage under my spouse's plan. Can I decline Enterprise coverage?
Yes, you may elect to waive coverage by checking the WAIVE box on your Benefits Enrollment Form. Be sure to supply the source of other coverage and return your signed form to your local HR department within the required timeframe. Keep in mind that you can still enroll to participate in Flexible Spending Accounts and supplemental life insurance even though you do not participate in the Enterprise health care plan.
Can I enroll in an EPO and select PPO coverage for my dependents?
No, you and your dependents must enroll in the same medical plan. (Exception: If you and your spouse are both employed full-time by Enterprise, each may enroll separately and into different plans.)
What if I select the EPO and then change my mind?
You are committed to your enrollment choices for the entire plan year (Jan. 1 - Dec. 31) and may only change from one plan to another during open enrollment which takes place each year in the fall. Changes made during open enrollment become effective January 1.
Can I add or drop health care coverage during the year?
Certain life-changing events occurring throughout the plan year, such as marriage, birth or divorce, may allow you to add or drop coverage for eligible dependents within your current plan. To make a change, you must contact your local HR department in writing by e-mail, letter, or fax within 31 days of the life-changing event to complete the notification process. Once your local HR department has documented your notification, a Benefits Enrollment Form will be sent to your home address so you can complete the enrollment process
Can I designate more than one beneficiary for Basic Life, Optional Life, and AD&D?
Yes, more than one person may be selected as your beneficiary. However, you must designate the same beneficiary(ies) for Basic Life, Optional Life and AD&D. Basic Life/Optional Life/AD&D Beneficiary Designation Form (PDF)
Do I need to select a beneficiary for Dependent Life?
No, beneficiary designation is not required for Dependent Life (spouse life/child life) since the employee is always the beneficiary.
How do I update my Retirement Savings Plan beneficiary(ies)?
To set up or update Retirement Savings Plan beneficiary information, log on to NetBenefits® at www.401k.com. Click "Your Profile," then click the "Beneficiaries" link in the "About You" section. If you do not have access to NetBenefits® via the Web, you may also call Fidelity at 1-800-835-5095 and request a beneficiary designation form.
How do I get replacement ID cards?
For medical ID cards, call UnitedHealthcare at (800) 520-0746 or go to www.myuhc.com. For EyeMed Vision Care, call (800)841-1220. For prescription drug ID card, call Express Scripts at (866)291-2607. Delta Dental ID cards are available from your local HR department.
How do I check to see if my doctor is in the UnitedHealthcare network?
To learn if your physician is a UnitedHealthcare provider, call UnitedHealthcare at (800) 520-0746 or visit www.myuhc.com., enter your User Name and Password and go to Provider Information to conduct a provider search. Always verify with your physician that he/she is in the UnitedHealthcare network and accepting new patients.
What will my medical benefits cost me? (Medical, Dental, Prescription Drug Program)
A portion of the health care premium is paid by the employee and varies according to which plan you select and how many dependents are covered under your plan. The rates below are biweekly and payroll deducted pretax:

I'm getting married soon. Can I add my spouse to the medical plan?
Yes. To enroll your new spouse for benefits, you must first contact your local HR department in writing by e-mail, letter, or fax to complete the notification process within 31 days of the marriage. You will also need to submit a completed Benefits Enrollment Form and a copy of the marriage certificate within the time allowed to your local HR department. Benefits for your new spouse become effective on the first day of the next full benefit coverage period after you contact your local HR department to complete the notification process. Changes to premium deductions begin in the first full paycheck after the coverage effective date.
How will Flexible Spending Accounts (FSA's) for Dependent Care and/or Health Care save me money?
You can reduce your taxes and increase your spendable income by setting aside pretax dollars in a Dependent Care Spending Account and/or Health Care Spending Account. Through these accounts you can pay for qualifying dependent care expenses up to $5,000 annually and/or qualifying health care expenses up to $3,000 annually. Enterprise pays all administrative costs for this benefit.
First Time Enrollment Questions
What if I lose my Benefits Enrollment Form?
Contact your local HR department immediately. They will generate another Benefits Enrollment Form and mail it to your home address within 2 business days
What if I don't return my Benefits Enrollment Form by the due date printed on the form?
If you do not enroll and select your choice of medical plans within the time allowed, your coverage will automatically default to single (employee only) coverage through UnitedHealthcare's PPO600 Plan which is packaged with dental coverage through Delta Dental and prescription drug coverage through Express Scripts.
How do I decline medical coverage?
To decline medical coverage, check the "Waive" box on your Benefits Enrollment Form, indicate the source of other coverage in the space provided, sign and return it to your local HR department by the due date printed on the form.

When will I get my ID cards?
UnitedHealthcare and Express Scripts will mail your personalized ID card(s) to your home address shortly before your benefits go into effect. Delta Dental and Express Scripts prescription drug ID cards are available from your local HR department. EyeMed Vision Care will mail ID cards to your home address if you elect vision care benefits.
When will I be covered? (Medical, Dental, Prescription Drug, Vision)
Coverage under your Medical, Dental, Prescription Drug and Vision plans begins on the first day of the third consecutive month following your first day of work as a full-time employee. For example, if you begin work on April 11, your coverage begins on July 1. The same goes for any dependents that you include on the health care plan.

Open Enrollment Questions
Why is it important to participate in Open Enrollment each year?
Enterprise is committed to offering flexible plan choices and allows for you to make changes once a year during open enrollment. It is important that you carefully re-evaluate your options and make appropriate benefit elections that meet your needs for the coming year. Benefit choices made during open enrollment remain in effect for the entire plan year (Jan. 1-Dec.31).
What if I don't return my Benefits Enrollment Form during Open Enrollment?
Here is what will happen.
1) You will automatically be re-enrolled in the same medical plan and coverage level that you selected for the previous plan year (if that plan is still available in your coverage area*); *If the medical plan you were enrolled in for the previous plan year is no longer available in your coverage area, you will default to a similar plan at the same coverage level as the previous year.
2) any supplemental life coverage that you selected for the previous plan year will remain in force;
3) premiums for these plans will be deducted from your paycheck beginning Jan. 1st; and
4) enrollment for EyeMed Vision Care will be waived; and
5) since annual enrollment is required for Flexible Spending Accounts, you will not be able to participate in dependent care or health care spending accounts.
I've decided to enroll in my spouse's medical plan and cancel my Enterprise coverage. What should I do?
To cancel your Enterprise coverage, be sure to check the WAIVE box on your Benefits Enrollment Form, supply the name of your spouse's coverage as source of other coverage, sign and return the Benefits Enrollment Form to your local HR department by the open enrollment deadline. If you do not return the Benefits Enrollment Form by the deadline indicating WAIVE coverage, you will automatically be re-enrolled in the same medical plan and coverage level that you selected for the previous plan year (if that plan is still available in your coverage area*) and you will be required to make premium payments for the entire plan year. Keep in mind that you can still participate in the Flexible Spending Accounts and enroll in supplemental life insurance even though you do not elect Enterprise's medical coverage.
*If the medical plan you were enrolled in for the previous plan year is no longer available in your coverage area, you will default to a similar plan at the same coverage level as the previous year.
I'm keeping the same coverage as last year. Do I need to return the Benefits Enrollment Form?
Yes. All employees must complete and return their Benefits Enrollment Form to ensure that appropriate benefit elections are made. Remember, the choices you make now remain in effect for the entire plan year (Jan 1-Dec 31) and cannot be changed unless you experience a qualifying life-changing event.
How do I select a Primary Care Physician (PCP)? Selecting a Primary Care Physician (PCP) is required for employees and their dependents enrolling in the UnitedHealthcare EPO for the first time. To secure a PCP ID Number for EPO enrollment or to learn if your physician is a UnitedHealthcare provider, call UnitedHealthcare at (800) 520-0746 or visit www.myuhc.com, enter your User Name and Password and go to Physicians & Facilities to conduct a provider search. Always verify with your physician that he/she is in the UnitedHealthcare network and accepting new patients. (A Primary Care Physician (PCP) is considered to be one of the following: Internist, Family Practice Physician, General Practitioner, and Pediatrician. All other types of physicians including OBGYNS are considered specialists.)
My wife and I both work for Enterprise and we both participate in the optional life insurance plan. Can I also purchase dependent life insurance for her?
No. If you and your spouse are both employed by Enterprise, you may not cover your spouse as a dependent, your spouse may not cover you as a dependent, and only one parent may cover the dependent children.
