Benefits eligibility depends on your situation as described below:
I am a part-time new hire at Enterprise
Part-time employees are eligible for Kaiser Permanente (medical, dental, drug, optical) coverage and EyeMed Vision Care benefits effective the first day of the month following 1st day of work. What does that mean? If you started anytime in September, your Kaiser Permanente and EyeMed benefits will begin on October 1. Be sure to complete and return your Benefits Enrollment Form to your local HR department by the due date printed on the form.
I am a full-time new hire at Enterprise
Full-time employees are eligible for Kaiser Permanente (medical, dental, drug, optical) coverage and EyeMed Vision Care benefits effective the first day of the month following 1st day of work; and eligible for life, AD&D, LTD and FSA effective the first day of the third month following 1st day of work as a full-time employee. What does that mean? If you started anytime in September, your Kaiser Permanente (medical, dental, drug, optical) and EyeMed benefits will begin on October 1 and your life, AD&D, LTD and FSA begins on December 1. Be sure to complete and return your Benefits Enrollment Form to your local HR department by the due date printed on the form.
Eligibility for the Enterprise Retirement Savings Plan
Full-time and part-time employees are eligible to participate in the Retirement Savings Plan as follows:
- 401(k) employee contributions/employer match: 1st day of 3rd month of continuous employment
- Profit Sharing: 1st day of month following one year of continuous service and 1,000 hours of service in your anniversary year
Enroll in the Retirement Savings Plan online via the Fidelity NetBenefits® Web site at www.401k.com or call 1-800-835-5095.

Birth, Adoption or Placement for Adoption
To enroll your new child for benefits, you must first contact your local HR department in writing by e-mail, letter, or fax within 31 days of the birth, adoption or placement for adoption to complete the notification process. Next, you will need to complete and return a Benefits Enrollment Form and a copy of the birth certificate to your local HR department within the time allowed. Benefits for your new child become effective as follows: Kaiser Permanente (medical, dental, drug, optical) benefits, EyeMed Vision Care, and child life insurance benefits go into effect on the date of the birth, adoption, or placement for adoption; flexible spending account changes go into effect on the first day of the next full benefit coverage period after the birth, adoption, or placement for adoption.
Marriage
To enroll your new spouse for benefits, you must first contact your local HR department in writing by e-mail, letter, or fax to complete the notification process within 31 days of the marriage. You will also need to submit a completed Benefits Enrollment Form and a copy of the marriage certificate within the time allowed to your local HR department. Kaiser Permanente (medical, dental, drug, optical) and EyeMed Vision Care benefits for your new spouse go into effect on the first day of the month after you contact your local HR department to complete the notification process; dependent life insurance (spouse and child) and flexible spending account benefit changes go into effect on the first day of the next full benefit coverage period after you contact your local HR department to complete the notification process. Payroll deductions begin on the first full paycheck after the coverage effective date.
Part-time to Full-time
If you have changed from a part-time employee to a full-time employee and you are already participating in the health care plans, your Kaiser Permanente (medical, dental, drug, optical) and EyeMed Vision Care benefits will remain the same, however, you will be eligible for life, AD&D, LTD and FSA effective the first day of the third month following your first day of work as a full-time employee. What does that mean? If you went full-time anytime in September, your life, AD&D, LTD and FSA benefits will begin on December 1.
It's Open Enrollment for the next year's benefits
Changes to your benefits become effective on January 1.
